You can't save a paint as a .car file in any painting program, the .car file is created in game via the opponent manager.
In photoshop merge all the layers into one single layer flat. Then save that flat as a .tga (Truevision Targa) file in the "exports/imports" folder that is located in the main NR2K3 folder. Then start the game, go into the "Opponent Manager" and choose "New", select the model of the car that your completed paint will go on, Chevy, Ford, etc., and give the file a name. You should then have a blank grey primered car. Choose the "Import" button and the .tga file you saved from photoshop should be visible (in name only), select that file and it will then be applied to your primered car. Give the driver a name and a car number and select "Save". Do the same thing under the "Crew" tab to apply any crew you might've painted too. The new car should show under the roster side of opponent manager. Be sure you check the box beside the new car to add it to the roster. Then exit opponent manager and select "Player Info", click the arrow until you see the new car and that will set it as the car you will drive in the race. Some guys also create .car files with WinMip2 but to me the opponent manager method is just as easy and lets me see my finished car in-game right away. Hope that helps!

save it as a TGA file in your imports/exports folder. In the opponent manager menu make a new car or use an existing one and click the import button. From there select your TGA file and viola there you have it!
