Think this link will work. It's from NextelRacer @ NNRacing.com...I use it to create the Conway numbers, but I believe they have the # 34 posted at MasGrafx.

I think you'll have to squish & skew those, but yes those are the proper ones. If you need any help adjusting those numbers, let me know
